Waiver & release of liability
ASSUMPTION OF RISK, WAIVER AND RELEASE OF LIABILITY
I, the participant (or the parent or legal guardian of a participant under 18), want to take part in programs run by High Performance Goaltending ("HPG"). These include lessons, camps, clinics, training sessions, team activities and related events. In exchange for being allowed to participate, I agree to the following:
Risks. Ice hockey and goaltending training are physical activities with built-in risks. These include being hit by pucks, sticks, skates or other players, falls on the ice, collisions with boards or nets, muscle and joint injuries, concussions, and in rare cases serious injury or death. I understand these risks and voluntarily accept them.
Equipment and health. The participant will wear full, properly fitted protective goaltending or hockey equipment at all times on the ice. I confirm the participant is physically able to take part and has no medical condition that would make participating unsafe, except anything I have told HPG in writing.
Release. To the fullest extent allowed by law, I release and agree not to sue HPG, its owners, coaches, staff, volunteers and the facilities it uses for any injury, illness, loss or damage connected to participation. This covers claims caused by ordinary negligence. It does not cover gross negligence or intentional misconduct.
Medical treatment. If I cannot be reached in an emergency, I allow HPG staff to get emergency medical care for the participant. I accept responsibility for any resulting costs.
Rules and conduct. The participant will follow coach instructions and facility rules. HPG may remove a participant for unsafe or disruptive behavior.
Photos and video. I allow HPG to record and use photos or video of the participant for training feedback and HPG promotion, unless I tell HPG in writing that I opt out.
Governing law. This agreement follows Wisconsin law. If any part of it is found invalid, the rest still applies.
I have read this waiver, I understand that I am giving up certain legal rights, and I am signing it voluntarily. If I am the parent or guardian, I sign on behalf of myself and the participant.
